Key Responsibilities:
Provide skilled nursing care to elderly patients, including medication administration, wound care, vital signs monitoring, and management of chronic illnesses.
Conduct patient assessments and develop individualized care plans tailored to each senior’s needs.
Assist with activities of daily living (ADLs) such as mobility, hygiene, nutrition, and medication reminders.
Educate patients and their families on healthcare management, preventive care, and lifestyle adjustments.
Coordinate with doctors, physiotherapists, and other healthcare professionals to ensure seamless care.
Maintain accurate patient records and reports in compliance with healthcare regulations.
Monitor patients’ physical and emotional wellbeing, providing companionship and support.
Respond promptly to emergencies, ensuring patient safety at all times.
Qualifications:
Valid Nursing License (DHA, HAAD, or MOHAP as applicable).
Minimum 2 years of nursing experience, preferably in geriatrics, homecare, or hospice.
Knowledge of geriatric health issues such as dementia, diabetes, hypertension, and mobility limitations.
Strong communication, empathy, and interpersonal skills.
Ability to work independently and manage patient care with professionalism.
Certification in Basic Life Support (BLS); Advanced Life Support (ACLS) preferred.
